SABBATIC POLICY, non-academic

I have heard a sabbatic policy for non-academic employers is popular in some
parts of the world. This sounds humane and appropriate to me. Does anyone
have a sample of such a policy or do you know the terms of the leave? In
particular:
- Benefit and pay policy during the sabbatic,
- Whether work or research is required,
- How long is the leave, how often,
- What is the employment period to qualify,
- Who approves and schedules sabbatics
